As one of seven centers at Indiana University’s Institute on Disability and Community, we do research and provide training, assistance and information to individuals, families, professionals and communities. We work in partnership with adults who are aging with disabilities to promote their well-being, community-participation, self-determination and leadership.

Our staff is insured, bonded and has a criminal background check done ensuring that a knowledgeable, dependable and trusted staff member is providing care to you or your loved.Understanding Geriatric Care Management* Conduct in-home assessments – Identify formal and informal support networks; assess physical abilities, psychological status, health care needs, home safety, social activities, financial and legal preparations.* Meet immediate needs, coordinate day-to-day care, escort to medical appointments, document medical information, provide support with complex needs.* Work in conjunction with home care providers such as Comfort Keepers to arrange, coordinate, and monitor in-home services* Offer counseling and support to Elders in adjusting to their changing needs; and to families in managing the care of their loved ones* Serve as liaison to families who live at a distance* Provide advocacy, guidance and support for future planning* Assist with finding alternative housing and the process of moving, when needed* Offer referrals to needed professionals in the community with expertise in Elder law, financial advising, etc.Ann Kerr RNGeriatric Care Manager

Experience Works, formerly known as Green Thumb, is a national non-profit organization whose mission is to improve the lives of older people. Achieved through training, community service and employment, hiring these pre-screened participants makes good business sense. On average, not only do older workers have lower absenteeism, they tend to stay on the job longer than younger workers. Experience Works staff matches qualified mature workers with local businesses who seek reliable, experienced employees. To encourage hiring these mature workers, Experience Works can offer eligible for-profit businesses up to 50% reimbursement of wages to employers during a training period.To be eligible for program services, applicants must be 55 or older, unemployed, at an income below 125% of the federal poverty income level, and living within the county serviced by Experience Works.
